What We Look For in a Hospital Case Manager:
RN or LSW
2-3 years Hospital Case Management experience with some type of critical care/medically complex case background with-in the last 3 years
More Insight of Daily Responsibilities of a Hospital Case Manager:
In this opportunity, you will have 15-20 patient caseload. You are responsible for everything from start to finish so although 15-20 doesn't seem like that much, this is extensive. You will handle discharge planning, care conferences, meeting with families, interdisciplinary planning/management, Utilization Management, Financial Case Management, etc.
